What is Gynecomastia?
Gynecomastia, a medical word from the Greek words for “woman-like breasts,” affects ten percent of men. The condition generally only lasts for a few years for boys going through puberty, and is typically resolved by the time they turn 18. However, this condition can be much more uncomfortable for an older man, causing self-esteem and self-confidence issues even though it is not life threatening.
The important thing to know is that you are not alone. 36% of adult young men and 57% of older adult men in the United States have this medical condition; in fact, about 200,000 to 3 million cases of gynecomastia are being reported every single year
What Causes Gynecomastia?
Gynecomastia is often associated with medicinal causes, obesity or hormonal metabolism imbalance. Too much estrogen present in the body can cause breast growth; so can steroid or drug use. Some studies have linked hormones used in livestock production – including beef and dairy products – to breast growth in boys and young men, which does not go away after puberty.

If there are simply fat deposits around the chest area, liposuction may be all that is needed. If there is a circular disc of enlarged breast tissue caused by a hormonal or glandular issue, however, surgical intervention may be required.
The Male Gynecomastia Procedure
Male gynecomastia is performed by Dr. Man in our Boca Raton plastic surgery facility, under general anesthesia administered by an MD anesthesiologist. The procedure can last from one hour to three hours depending on the extent of the correction.
If your gynecomastia is primarily caused by fatty tissue, liposuction is usually the procedure of choice. Liposuction surgery is a procedure used to gently remove unwanted localized deposits of fat cells from the body.A cannula is inserted through a tiny incision placed in an inconspicuous location in the skin. Using a vacuum system attached to the cannula, excess fat cells are gently loosened and sucked out. The result is a resculpting of a bulging, fatty breast area into more attractive contours.
Traditional surgical excision may be a better option if the gynecomastia is formed of more breast tissue than fat deposits. The excision may be performed alone or with liposuction. Typically, a small incision is placed on the edge of the areola, or in the underarm area. Dr. Man will work through these incisions to carefully remove the excess glandular tissue, fat and skin from the breast.
Many plastic surgeons use a larger incision for patients with significant amounts of fat and breast tissue, but Dr. Man has developed a combination treatment method for male gynecomastia that uses Tickle Lipo. This is an advanced form of liposuction and laser therapy which provides an innovative way to cause skin retraction without a need for a large excision, preventing large scars.
During Gynecomastia Surgery
Most gynecomastia procedures will take between two and four hours. While many surgeons do gynecomastia solely as an outpatient procedure at a hospital, ambulatory facility, or office suite, Dr. Man has created a center that provides patients with complete care including the option to recover under the watchful eye of credentialed staff.
Prior to the actual surgery, you will be given your pre-op medications to keep you comfortable. The anesthesiologist will normally administer general anesthesia so you will be asleep and not experience any sensation during the procedure (local anesthetics and / or IV sedation may also be used.)
Dr. Man will complete your surgery following the surgical plan which was pre-determined in your consultation and verified before surgery. Once the surgery has begun, slight changes in technique may be made based on the condition of the breast tissue and elasticity of your skin.
After the reduction is complete, sutures will be used to close the incisions, and in some cases drains may be placed to prevent swelling from excess fluid. Surgical dressing in combination with a special support garment will be used to support and protect your chest.
Safety measures during surgery include multiple monitors in the surgical suite being sued to monitor your blood pressure, heart rate, pulse, and oxygen levels. After your surgery, you will be wheeled to a recovery area, where you will continue to be closely monitored until your anesthesia wears off.
After Gynecomastia Surgery
After your procedure, you will be allowed to rest comfortably until your anesthesia begins to wear off. Any necessary medications will be administered to prevent infection and minimize your discomfort. You will need someone to be by your side for 24 hours after the surgery. You can always opt to stay and be cared for during the first 24 hours by our well trained, compassionate staff.
Recovery from Gynecomastia Surgery
Bed rest is recommended for the first day. You will experience some discomfort for several days. For the first few days after surgery you will wear surgical dressings which will be exchanged for a special compression garment. This will be worn for about 2 weeks.
Most discomfort associated with gynecomastia surgery can be controlled with oral medication prescribed by Dr. Man. Antibiotics may also be prescribed. Most men experience some degree of swelling, bruising and tenderness. Most patients are back to work within the first week or two. Following your gynecomastia surgery, you will enjoy a flatter, more masculine chest shape.
